We participate with most major insurance plans. Since managed care plans change frequently it is important to make sure that we have correct insurance plan information. In order to use your insurance benefit, without extra out-of-pocket expense, it is your responsibility to notify us of any changes in your plan. Please present your insurance card at the time of check-in for your appointment. You are responsible to know the details (e.g. co-pay, deductible, etc.) concerning your particular insurance. As a courtesy, we will file claims for our services with your insurance company.
Fees are expected to be paid at the time of service. We accept cash, check, Visa, MasterCard, or Discover. If you have an insurance co-pay, you must satisfy this requirement before you leave the office. A service fee will be assessed if we have to bill you for your co-pay. If this policy presents an undue hardship to your family now, or at any time in the future, please ask to speak to a patient representative to discuss your situation.
