Please bring your insurance card to each visit.

Also, refer to our Managed Care Plans for a listing of insurance
plans with whom Ottawa Medical Center physicians participate.
On the first visit to Ottawa Medical Center, you will be asked
to fill out a patient information sheet.

For an urgent appointment
when the patient needs to be seen as soon as possible please ask to speak with the nurse assigned to your physician.

Every effort will be made to have your refill completed within 48 business hours.
